If a member desires to start a new ministry, then they need to fill out this application. 

For a volenteer to be involved in Children's Ministry a criminal background check is nessessary.  If you print off an application for the purpose of getting involved in children's ministry, then you will need to fill out this form so we can have your approval to run a criminal background check.
